Chronic Disease Management (CDM)

Who is eligible?

You are eligible to join this programme free of charge if you:

  • Hold a valid Medical Card or GP Visit Card

  • Have one or more of the following chronic conditions:

    • Type 2 Diabetes Mellitus

    • Asthma

    • Chronic Obstructive Pulmonary Disease (COPD)

    • Cardiovascular disease, including:

      • Heart failure

      • Previous heart attack or angina

      • Stroke

      • Atrial fibrillation

  • aged 18 years or older (eligibility from the start of 2022)

Patients aged over 70 with a valid Medical Card or GP Visit Card and specific medical conditions will also be monitored under the Chronic Disease Management Programme.

The practice will contact you if you are eligible.

How does the programme work?

  • Two structured reviews will be provided each year at the GP surgery.

  • Each structured review consists of:

    • One visit to the practice nurse (including any required blood tests)

    • Follow-up visit to the GP at the surgery
